Belt Grading Booking Form

I understand the following

10th Gup students (all ages) require Dipped Foam Shin Guards.

10th Gup students (11 Yrs+ ) require a Tanto.

Males also require a Groin Guard.

9th Gup students require a complete set of sparring pads

(Females: Hand & Foot guards, Males : Hand, Foot & a groin guard).

6th Gup students require a Bo Staff & Bo Case.

4th Gup students require safety Nunchaku.

2nd Gup students 8 - 10 Yrs of age require a Tanto.

I understand the following

Students Late to the Grading will not be permitted to join the grading; a free next grading availabily will be offered.

Gradings may overrun; please allow an extra 30 minutes to the scheduled grading time.

Students must have a clean and presentable uniform. Trouser hem and Jacket cuff turn-ups are not permitted.

10th Gup+ students are required to have their TEMAS patch sewn appropriately on their uniform.

Students who do not show up to their scheduled grading will not be refunded their grading fee and will be require to rebook and pay.

Students who cannot attend their scheduled grading and contact SHMA a minimum of 30 minutes prior the their scheduled grading time will be offered a 2nd grading free of charge.
